General Terms and Conditions (GTC) for the Physiotherapy Clinic of Mgr. Lucie Fričová
According to § 1751 para. 1 et seq. of Act No. 89/2023 Coll.
1. INTRODUCTORY PROVISIONS
These general terms and conditions regulate the conditions for providing healthcare services by the physiotherapy clinic of Mgr. Lucie Fričová (hereinafter referred to as the "clinic"), located at Žitavského 497, Prague 5 - Zbraslav, 15600, ID: 72063963, registered in the trade register.
2. SERVICE RESERVATIONS
Clients reserve appointments via telephone, email, online form, or directly at the clinic according to available times.
By submitting a request for appointment reservation, the Client expresses agreement with these conditions and undertakes to abide by them.
At the first visit, the Client must present correct and current health information including medical reports. If care is to be covered by insurance, it is necessary to bring a valid physiotherapy voucher. This voucher is valid for 3 months from issuance - after this time, unused therapies can no longer be utilized. A new voucher can be requested from a doctor.
At the first visit covered by insurance, we collect a 300 CZK cash fee for appointment reservations. If you attend the agreed appointments or cancel them at least 24 hours in advance, we will refund this fee during your last therapy session.
The clinic is obligated to inform the client in advance about the number of therapies it can offer due to capacity and operational reasons. The number of therapies may differ from the number on the referral. The clinic undertakes not to change this number during the course of therapy. Only by agreement between the therapist and client can the number of therapies change. Both parties must agree to the change and the reason for the change must be noted in the daily record.
The clinic may refuse to provide care in these cases:
- Necessary information about the Client's health condition cannot be obtained
- Health reasons arise that make care impossible
- The Client does not meet or violates clinic conditions
- If accepting the Client would threaten the quality and safety of care due to staff overload or insufficient equipment
3. PAYMENT FOR SERVICES
The clinic has contracts with insurance companies VZP, OZP, ZPMV ČR, ČPZP, RBP, VoZP and also provides services for direct payment.
Current prices are listed in the price list on the website www.fyzio.net.
Price changes take effect on the first day after publication of the new price list. Adjustments do not apply to already reserved services with issued invoices or paid deposits.
You can pay in cash, by QR code after the visit, or in advance by invoice - at the latest by the therapy date. Without payment, the reservation will be cancelled unless otherwise agreed with the clinic.
4. APPOINTMENT CANCELLATION RULES
When a Client finds they cannot attend the agreed appointment, they must notify the clinic as soon as possible: SMS to 773 469 848 or email to ordinace@fyzio.net (including weekends and holidays).
For self-paying clients:
If the client cancels more than 24 hours before the scheduled service appointment - the cancellation fee is 0% of the price, even if the price has already been paid.
If the client cancels less than 24 hours before the scheduled service appointment or if the client does not show up for the scheduled appointment - the cancellation fee is 100% of the agreed therapy price. The clinic is entitled to offer the Client another service appointment only after payment of the required price.
For insurance-covered therapy:
If the client cancels more than 24 hours before the scheduled service appointment, the clinic will offer the Client an alternative appointment.
If the client cancels less than 24 hours before the scheduled service appointment or if the client does not show up for the scheduled appointment, the therapy appointment is forfeited without replacement. As compensation necessary for clinic operation, a 300 CZK appointment reservation fee is deducted.
Additional conditions:
If the clinic must cancel an appointment (therapist absence, organizational reasons), it will offer an alternative appointment as soon as possible.
Gift vouchers are valid for six months from purchase.
5. THERAPY PROCESS
The Client agrees that the reserved time includes, in addition to the therapy itself, also preparation of space, equipment, administrative tasks of the therapist, creating videos, photos and writing exercise instructions for home practice, so that the therapy has the greatest benefit.
The clinic may refuse or prematurely terminate service without compensation or refund in these cases:
- Client concealed infectious or other serious illness
- Client provided incorrect, incomplete or outdated information
- Client concealed important information about their health
- Client repeatedly violates conditions even after warning
- Client arrived in a condition that does not allow safe service provision for hygienic reasons
- Client came under the influence of alcohol or drugs
Photography and recording:
- Client may photograph or record only with the therapist's consent
- Client must not photograph or record the therapist without her consent
- Therapist may make records for treatment documentation
- Records may be published only with Client's consent
Additional rules: The Client has no right to a specific therapist. The clinic strives to maintain the same therapist throughout treatment but cannot guarantee this (illness, job change, etc.). Presence of other persons (except legal guardians) requires therapist consent.
6. CLIENT RIGHTS AND OBLIGATIONS
The Client is obligated to:
- Provide truthful and current information about themselves and their health, report changes immediately
- Follow therapist instructions
- Maintain basic hygiene
The Client has the right to:
- Be informed about reasons for recommended therapy, effects and risks
- Access their medical documentation
- Ask about their health condition and provided services
- Request proper billing
- Refuse presence of uninvolved persons (students)
- Complain about service defects - must do so immediately upon discovery
Additional provisions:
For minors, the right to information belongs to both legal guardians and the Client themselves - according to their age and ability to understand.
Use of therapist services outside the clinic is prohibited.
In case of violation of these conditions, the clinic may refuse or terminate services without compensation.
